Name of Wine/Cider: Formal Name/Title (as it appears on bottle/label)
Appellation: Where the fruit is grown
Wine/Cider Grown & Made by Wine/Cider Maker: select “Yes” if the fruit was grown on company’s property
Sweetness: enter one or the other (% RS OR grams per liter)
Total Acidity: enter one or the other (grams or litre tartaric)
